    This is an example of why we need to be open about mistakes we make. If the first mistake was made known to the staff, perhaps the second incident would not have occurred....It also points out the importance of nurses knowing the actions of the meds they are giving...Then maybe neither of these mistakes would have happened....If I am not sure about a med I stop and look it up... This was 2 tragic mistakes....

    The sad thing is, the doctor who had no involvement in this is the one likely to get sued, right along with the hospital and the nurses. This is tragic.
