Coverage Overview


Who is eligible for this coverage?


This coverage is available to nurses under age 70, your lawful spouse and unmarried, dependent children, typically at least 15 days of age, but under age 19 (25 if a full-time student) (subject to state variations).

Who receives the benefits?


Your benefits go directly to you or to your beneficiary, not to a doctor or hospital.

Benefits are paid regardless of any other insurance coverage you may have.

What does this insurance cover?


The plan pays a benefit as shown in the charts below if a covered accident results in death within 90 days of the accident.

Additional Benefits


Common Carrier benefit: If loss of life is caused by an accident that happens while the primary insured or spouse is a fare-paying passenger in a public conveyance operated by a licensed common carrier, an additional amount equal to the benefit amount payable for loss of life will be paid. This benefit applies to primary insured and insured spouses only.

Seat belt benefit: If an insured person dies within 90 days as the result of an injury caused by an automobile accident, and was properly wearing a seat belt at the time of the accident, this benefit will pay an additional 50% of the person's full benefit amount or $50,000 (whichever is less).

Education benefit: If your coverage includes dependent children and you lose your life in a covered accident, this benefit will pay an additional $5,000 per year for up to four years to any eligible insured dependent child enrolled as a full-time student in an institution of higher learning, or who becomes enrolled within 365 days of your death.

Injury: If a covered accident results in dismemberment within 90 days of the accident: The plan pays the same amount as the basic death benefit for loss of sight of both eyes, both hands, both feet, or any combination of foot, hand, or sight in one eye. The plan pays half the amount of the death benefit for loss of one foot, one hand or sight in one eye.

What if I enroll, then change my mind?


You'll have a full 30 days after you receive your Certificate of Insurance to make sure you're completely satisfied with your coverage. If you change your mind, you can return your certificate within 30 days and get a full refund of any premium paid.
