Are the abbreviations for pulse and after with undercase or uppercase p's????

Do you mean TPR? T-98, P-16, R-20? I have always used them as caps, but have seen them as lower case too My advice to you is to not depend on using abbreviations because I know in the western states, more and more hospitals are only allowing a very few abbreviations to be used.

CAPS for TPR. for after, lower case. I have almost quit using any abbreviatiions.

We have been instructed to use as few abreviations as possible more med errors have occurred because of misinterpreted initials. A revised list of approved abreviations is supposed to come from our Board of Nursing later this year.

I am with cdnurse on this one, I don't use them much, some abreviations I see used drive me nuts, last week I saw an abreviation of H/A I had no idea it was hearing aid. another example is F/C use only standardized abrievations, or your facilitys standard abreviation list, oh by the way F/C is foley catheter.

We need to keep some abbreviations. We use them to speed our charting. I would hate to have to write out Temperature every single time I took vitals. What we need is a national standard, and I think Joint Commission is moving in that direction, by default anyway.

What would really help is better penmanship (myself included, alas).
