It seems to be on the rise. I haven't had any personal experience, but I just read about this incident in my birth state - West Virginia. I think that nurses and doctors seem to be involved in these sorts of issues equally. I think that more often than not nurses are asked to take the fall for it though. Doctors never know how to admit when they're wrong - EVER!
