A 37-year old male with a history of schizopherenia is having auditory hallucinations. The physician orders 200 mg of haloperidol , brand or trade name ( Haldol) orally or I.M. Q4hrs. PRN. What is the nurse BEST action. This question is for all the Psychiatric Nurse Geniuses, who loves Psychiatric Nursing!!!!!

That is funny dmazment, but that kind of therapy will be benefical for the nurse, not the patient.

Look at the amount of the medication, before you decide which route to use, remember the order states you can administer either I.M or Orally. Try again!!!!

I think the dose is too high. If he's not harming anyone or out of control, I wouldn't medicate him.

Char, you answer it partially, what should you do first, remember he is hearing things, and is displaying it severly, he needs to be medicated.

I wanted to comment in the sceniro that is base on a true event. My boyfriend lived next door to a male that had schizopherenia with auditory hallucinations, the male also, spoke in a different voice, so in his mind, he though the other person was physically present. To make a long story short, the man, thought the other person was going to hurt him, so he started getting very scared, and worked his self into a frenzy, my boyfriend heard two voices coming from his house, the man was saying please help me, please help, me, it sounded like someone was beating up the man, all of a sudden, everything became quiet. My boyfriend, ran upstairs, where the man was, and found him dead, he had worked himself into a fatal heart attack. So in cases where the patient, is severely paranoid, or is hearing voice, and acting it out severely, they need immediate medical attention, it is very vital to there own life.

haldol is used to treat and control mental disorders, primarily schizophrenia. yes the dosage is way to high, but first I would call the pharmacist to ask what the recommended dosage and frequency should be and then call the physician to have it changed from prescribed dosage to correct dosage, from prn to regular daily dose, usually two to three times per day.

The reason I'd call a pharmacist first is ammunition for what I think is the best for my patient and reinforce haldol is not usually used as a prn med for schizophrenia but a regular daily dosage for treatment

So that makes it the more funnier. I wanted to comment on the the haldol is not usually used as a prn med for schizophrenia. You are right, my half brother has schizophrenia, but he doesn't have the one with hallucinations, he is taking Seroquel orally 300mg daily at bedtime. I think in this case, the patient was really displaying auditory hallucination, which can be very dangerious, depending on the severity. Haldo, can be given hourly, as needed until control is obtained. Initially 0.5 to 5mg P.O b.i.d. or t.i.d. or, 2 to 5mg I.M lactate q4 to 8 hours. So you are right about the PRN order, so that's why the MD needs to be notified, to clarify the order, because Haldo can only be given hourly not PRN, and the Maximum dosage is 100mg P.O. daily, and for Chronic psychosis requiring prolonged therapy, . Adults : 50 to 100mg .I.M. deanoate 4 weeks. 200mg q4 as needed would be overmedicating the patient, possible overdosing the patient.
