Part of the NCLEX exam is to prepare the testers to think Critically. I will introduce a senerio. L. J is a 79 year old male, who had a abdominal surgury for colon resection. THe narocotic analegesic merepridne ( Demerol)/75 q3 to q4, was prescribed following the surgery. L.J. did not ask for "pain" medication" because he thought he might become addicted to the narcotic. The nurse noted that he was restless and grimacing when he moved in bed, he also refused to breath deeply and cough when instructed to do so.The nurse compared his vital signs to his baseline findings, his pulse rate had increased and his systolic blood pressure had decreased by 6mmg Hg. 1. Should the nurse give the Medication. 2. What would your reaction be to L. J. in regard to his restlessness, grimacing, and refusal to deep breath and cough. 3. What is the significance of the changes in the vital signs. 4. what classic side effects of narcotic analgesics should the nurse assess?. 5. L. J. does not want to ambulate, what is an appropriate nursing response? Last, What nursing diagnosis for this patient is appropriate.

I am also preparing for my LVN boards soon, Just have to register. I am using all this time to prepare, and practice. I want to nail that NCLEX the first time.
