Posted about 5 years ago Has anyone had to dael with this issue in a patient before? I have a client that has this including an extremely high arch. This was a hoem birth at mom's request, so, there was almost non-existent prenatal care. This child is 4 months old and has feeding problems. Are there any other issues that I need to make mom aware of? I am guessing that there will be speech delays due to the tongue being set so far back in the mouth, but what else? Thanks Guys! |

| Posted about 5 years ago I do not know for sure but I would make a guess that they may have problems with thier teeth. Let me see what I can find in my old books. Never run across this one before. |

| Posted about 5 years ago Found this for you: Micrognathia is a relative term describing the small size of the lower jaw. Considerations: In true micrognathia, the jaw is small enough to interfere with feeding of the infant. Infants with micrognathia may require special nipples in order to feed adequately. Micrognathia may be the only abnormality in a child, and is often self-correcting during growth, especially at puberty when the jaw grows significantly. It may also be associated with certain inherited disorders and syndromes. Micrognathia is one cause of abnormal alignment of the teeth . This can be seen by noticing the closure of the teeth. Often there will not be enough room for the teeth to grow and an orthodontist should be consulted when the adult teeth come in. At times, the child can outgrow micrognathia and it makes sense to wait until they are older. Common Causes:

| Posted about 5 years ago I knew you would come through for me, cd. Thank you! I did google this symptom, but your info is much better than I found. I am going to have to walk on egg shells with this mom. Mom actually called me 3 days after baby was born to have me do a home visit and help her with breastfeeding. When I went to her home, I find out that mom hade 5 prenatal visits during pregnancy. This is child #5 for her. Birth was attened by her husband and a "midwife", which I find out later that the "midwife" was just a mom in the community that had had her last 9 children at home. When I did my oral assessment I found the high arch. Baby had poor suck as well. Honestly, breastfeeding was out of the question. Baby did not have enough suck to cause let down. When I explained this to mom and offered fornula, mom was insistent that the "midwife" said everything was okay and the breastfeeding was going to take time. Mom also stated that the "midwife" did not tell her about the small jaw, poor suck, or high arch. The "midwife" siad everything was okay. I followed up in the home everyday for 4 more days, begging this momeach time to do formula or pumped breastmilk in a bottle for this baby. all the while, baby has lost over 2 pounds in 7 days after birth. Finally, after the 8th day, mom had the baby seen by a chiropractor and when the chiropractor adjusted the lower jaw to help with feed, baby started screaming and mom rushed baby to the doctor. Baby had a full work-up and was severely dehydrated (big suprise). Mom is now giving only formula, but is really reluctant to use "normal" medical pathways for care. She is allowing me to do case management home visit, so I am hoping with a little proactiveness on my part, I will be able to assess any problems long before they are severe. |
