tube feedings and scope of practice

Posted about 5 years ago I am currently a hospice nurse and am a RN. We had a patient that is toatally comatose and enrolled into hospice for sepsis from a sacral wound with osteomylitis. At this time her tube feeding was running at 90cc/hr through her g-tube. I got a call from our social worker who was at the facility. Patient is congested and becoming very edematous. To me she is no longer tolerating the tube feedings. In our IDG meeting previous we discussed weaning her off her tube feedings with the familys permission. The social worker did get an ok from the poa who is the mother. She handed me the phone so I could tell the patients staff nurse at the facility to turn the rate down to 45cc/hr. The plan was going to be to have a care conference the next day and I would inform the NP when I saw her and discuss the tube feeding rate. I am now in trouble at work for practicing outside my scope of practice for not obtaining a order from the np first before telling facility staff to cut the rate in half. I did not even think about it in this way. Can I be in serious trouble for this? I'm freaking out. What are the thoughts on this? |

| Posted about 5 years ago There wasn't an intent to harm and I assume there was no malpractice (injury). That means you're in pretty decent shape as far as keeping your license. However, you did go outside your scope of practice (you can only do things like that if you have standing orders in specialty units). I assume you're going to get a reprimand that will go on your record and you might be reviewed by your states nursing board but don't worry the sky isn't falling. |
