Posted almost 5 years ago Here are some adorable mix-ups in the chart I've recently seen: Pt scheduled for Knee Arthroscopy, pre-op diagnosis should be - Internal Derangement; actually read InterANAL Derangement (shall we schedule the GI doc to fix the scheduler's Cranial-Sacral-Transposition) Pt should have been scheduled for an Excision of Neuroma - actually scheduled for Excision of Aroma (very stinky feet!) What are your favorite charting Oops! ? |

| Posted almost 5 years ago I work 3rd shift so when I do my assesments, I see had 1st and 2nd have charted for their assesments. I saw that the 1st shift nurse had charted pedal pulse plus 2. The patient was a bilateral amputee. I wonder how she did that? This was from a seasoned nurse too. |
