We all know as part of the assessment or admission process we get to ask often times questions that we really don't want to know the answer to or the answer is completely shocking.....

I was hanging an IV and overheard another nurse 'Joann' doing an admission assessment with all the usal questions.  Apparently the report 'Joann Rn' had gotten from the ED was missing a few details, and lacked some of the pt's old history and old surgeries...

Joann to pt:    How much do you weigh?

Pt:  150lb

Joann to pt:  how tall are you?

Pt, (whipping back the sheets)  Uhm was that before or after I had the bilateral leg amputation.
