I thought I had seen some AH orders but yesterday I had the absolute worst. I had a 85 old woman admitted from ER with impaction and the order said "Give enema and have pt. hold for one hour." I was like where the hell am I going to put the enema cork!!! C'mon I know you have them too - share...

I was working the night shift. A patient was having difficulty with hypertension. Her BP was extremely elevated. The MD had been notified on several occasions of her elevated bp. Even though it was 3am, I felt I should notify him of this latest developement in the best interest of the patient and to cover myself. I called the MD and told him of the elevated bp. He replied "Stop taking her BP" and hung up!

Charlita, I LOVE that story! I have 2 stories: a very sleepy resident wrote one night to give "metameucil nebs." We laughed alot about that one, and ribbed the guy who wrote it from then on...it was so much fun. Then there was the time I called the doctor about 2 am about a patient's seriously declining respiratory status. He listened and then said he suggested I do this and that. I said, "What do you mean, you, 'suggest it. Is it an order or not.' He went on to tell me it was only a suggestion since he was not the patient's doctor. Opps--my bad.

Pittnurse, I am only a nursing student but this sounds like the way our text book would treat it. what did you end up doing to treat the pt.?

i was so tired during my shift. i have to monitor 2 behavioral prblems, monitor and put suppositories to 3 residents, call and follow up result and do charting and deal with phone calls as well as families. after 2 days, my adon called me to the office with the order that i wrote " apply suppository to the rectum now for constipation." i was so surprised and i started laughing while my adon just looked at me bewildered as she was holding my write up. While still grinning i said,' oh, i don't apply it i have to insert it.'

How about the dumbest order you have ever written? Nursing order or for NPs med order. A couple of weeks ago at the end of a long day I wrote 2 mg anxiety po. Thank goodness, I have a sharp nurse watching out for me.

I had a patient that was going through DT"s and was really combative. I called the MD he said well just give him .5mg Ativan, I told him that had already been done and was not effective. To which he promptly told me to give another .5mg. I told him I did not believe that would be effective either and that I was not interested in being a punching bag. He said "I don't think it is that bad" I told hime well you just come and see for yourself. He did not but did tell me to call him back in an hour. I did and every hour for the next 8. I made sure he did not sleep! Next time around he asked me what I wanted and got it!
