Our annual medicare/medicaid survey is in progress.It is very stressful!!To top it off,I entered a blood sugar of 55 in the MAR. My intervention was OJ with sugar,I failed to document this.The State  surveyor questioned me.I feel  bad,The DON said not to worry,it was OK.I keep hearing my  instructor in nursing school "If it wasn't charted,It wasn't done." I feel  stupid,I know I did the right things,But have no proof.We only had a


2.5% error rate in the med pass,which is very good,but I can't stop feeling bad...HELP

I don't think you should feel so bad about this. You know you did the right thing. We are all human and in the course of a crazy busy hectic day, like days when survey is there, we tend to forget things. I can't count the number of times i have gone home feeling like I missed something. And most of th time I haven't. It will be ok. that is a really good error rate by the way. So chin up! Don't beat yourself up about this.

Don't be so hard on yourself.  The fact that you got upset at yourself shows that you really care.  You are a hard worker and a great nurse.  Hang in there!
