Posted over 4 years ago Go to your states Board of Public Health Website: http://www.cdph.ca.gov/pubsforms/forms/Pages/LicensingandCertification.aspx Addresses for all CNA Registries: http://www.4cnas.com/id9.html
Many Police Departments, Mail Boxes inc. and the library offers the service for a few. 20.00 This fee will vary from store to store Department of Justice charged 51.00 This fee will vary from state to state Print Print out a copy of the Request for Live Scan Service to take with you to get your fingerprints scanned. SAMPLE FOR CERTIFICATION OF NURSE ASSISTANTS OR HOME HEALTH AIDES REQUEST FOR LIVE SCAN SERVICE One copy stays with the Scan Operator, one copy is for your files, one copy is sent to the address on the form. It took me one day to get Statis Confirmation that my scan was cleared. Call Department of Justice Statis Check Line (916) 227-4557 Use the ATI No. a the bottom of your form and your birthdate to confirm with their automated service. Often you will find a great amount of good information on the "Board of Nursing Website" but unfortunately you may have to search in the Department of Public Health in your state to find the actual Registry forms for CNA. I hope you found this helpful, send me a sign, rate this. Peace Rhianna Admiring the Struggle it takes to be Human |

| Posted over 2 years ago i just moved to niagara falls, new york. I thought i would have to take cna classes again. Turns out all i had to do was have a copy of my certification mailed here from missouri, which cost ten dollrs. Once i get that, i have to send it to the nrse admnstrn here n ny so they can switch my cert. Fifty dollars. Its all worth it, but its taking so long |
