I DIBN'T FEEL COMFORTABLE DOING THIS.....

Posted over 4 years ago I HAD A PATIENT WHO BEGAN HAVING ABDOMINAL PAIN. THE CHARGE NURSE CALLED TH E ATTENDING ON DUTY AND GOT AN ORDER FOR AN ENEMA. I WAS TAUGHT IN NURSING SCHOOL NEVER TO GIVE AN ENEMA TO A PERSON WITH THIS COMPLAINT. WAS I WRONG FOR NOT DOING THIS INITIALLY. THE PROBLEM WAS DONE IF I CAN RECALL AFTER AN ABDOMINAL X-RAY SHOWING CONSTIPATION. WAS I WRONG FOR NOT DOING WHAT THE ATTENDING ORDERED? DID ANYONE HAVE SIMILAR CASES? |

| Posted over 4 years ago SOME CHARGE NURSES DON'T WANT TO BE ACCOUNTABLE FOR THEIR NURSES I NOTICED. THEY JUST CALL THE DOCTOR AND STATE TO HIM THE SITUATION. ABDOMINAL PAIN OR DISCOMFORT CAN BE THE RESULT OF MANY THINGS AS YOU ALL KNOW. I WOULD NEVER FEEL COMFORTABLE BEING IN CHARGE.....I COULD NOT HANDLE THE PRESSURE. |

| Posted over 4 years ago I have been a nurse for 20 years and I always call the Md myself. I want to discuss MY PATIENT, with the Dr and if I have a question I ask it. If I would have been given an order for a enema I would have suggested a xray, most the time the Dr will go, Oh yea get an xray, or no give the enema. Then I would decide what I felt comfortable with. I did not start out with this confidence, but after many years you learn which MD's you can go, "Hey Dr. so in so, Mrs. so in so, is having abdomenal pain, can we get an Xray?" As far as charge nurses not backing you, then they can do it, if you do not feel comfortable. Just remember that is your Mom or Dad in the bed, and what would you want done if that is the case? Learn to always trust your gut... If you haven't been a nurse for very long, it will come, just keep trusting your gut reaction. Everyone I have ever talked to who had something go wrong with a patient, usually said they had a GUT feeling. (Obviously not in every situation, but you get my drift). Best of luck... | Posted over 4 years ago If I were in your case... I would look at the patient's chart for labs and assessment that could support the doctor's order. If there is no assessment, I'll perform it to determine a possible cause for the abdominal pain. If there is a contradiction in the order, I would ask the doctor like what Judleern said. |

| Posted over 4 years ago Ok now I'm confused. You said the X-ray had been done and it showed constipation and the dr ordered an enema. I should have asked was it a fleets or the old fashioned soap suds? Also I didn't see that you didn't personally have a chat with the dr. Why did you not speak personally with the dr? |

| Posted over 4 years ago NO HERE IS HOW THE STORY GOES. PATIENT C/O'S ''STOMACH PAIN. '' ( IT WAS NOT NAUSEA). I REPORTED THIS SOMEWHAT MODERATE PAIN IN THE PATIENTS ABDOMEN TO THE CHARGE NURSE. THE CHARGE NURSE CALLED THE DR.ON DUTY . THE DR. GAVE A VERBAL ORDER FOR AN ENEMA. HER LAST BM WAS NOT DOCUMENTED AND THE PATIENT WAS ELDERLY AND COULD NOT REMEMBER. I,IN A NICE WAY, SAID "NO I DO NOT FEEL RIGHT ABOUT THAT''. (GIVING THE ENEMA) THE DR. NEVER EXAMINED THE PATIENT NOR DID THE CHARGE NURSE. THE DR. JUST ORDERED THE WHATEVER KIND OF ENEMA. IT MAY HAVE JUST BEEN A POLICY OF THE HOSPITAL WHERE I WORKED THAT THE CHARGE NURSE ONLY TALKED WITH THE DR. SHE CALLED THE DR. BACK ABOUT THE NURSE REFUSING TO GIVE HER THE ENEMA. THE DR THEN CAME TO THE FLOOR AND EVALUATED THE PATIENT. THE DR. WAS BEAT RED MAD. |

| Posted over 4 years ago Since you re-explained things, then I have to say you did the correct thing. It could have been a bowel obstruction, aneurysm, appendicitis, ovarian cyst and a butt load of other things. I just want to say in your first post, it seemed the Xray had been done and showed constipation. I also want to say, I don't want to ever work where you do, because I'm responsible for my patients, as the assigned professional caregiver, not the charge nurse. This is a very old fashioned and dangerous policy, putting your license and the licenses of the other nurses in jeopardy. This kind of policy was popular way back in the 70's and earlier when docs were still considered the "captain of the ship" and should only be disturbed when absolutely necessary. |

| Posted over 4 years ago Not folowing orders can be tricky I think if a nurse is not comfortable with the order she should voiced her opinion. I had a case where a doctor ordered me to stop iv infusion and feed patient. In my judgement I was uncomfortable so I did not carry out the order, before the end of my shift the patient was off to OT. That was a gut feeling and assessment that work both in my favor and the patient.

Im confused, its been a long and tiring weekend! Joni.........your hospital states the charge nurse calls the md, not you? Geeeeeeez, that's just plain weird and dangerous. When I said to have the charge nurse back you up..........I meant that he/she be informed and stand by policy/procedure of the facility. We only have one heart, take care of it! Angie |

| Posted over 4 years ago Joni, I agree with you. I would not have done the enema either because there is so many other abdominal problems that that patient could have had. The doctor should have came and examined the patient before ordering the enema. |
