Hey everybody! I need some help coming up with strategies on preventing falls. What do you guys do at your hospital to prevent falls?

Our hospital uses a daily fall checklist that actually is done every shift to monitor every patient for their risk of falls if in the high catagory they have a fall bracelet and have an indicator on the outside of their door and on the Kardex. At least everyone will be aware who is a fall risk if it is not mentioned in report. The falls risk patients are close to the nurses station also.

When i was doing my clinicals at the hospital, they had color coded bars next to all the patient doors. Whatever fall category that patient was then that was the color used. Like orange was high fall risk and that bar would be out for everyone to see. Yellow was low risk and that bar would be out so everyone working would know which category each patient was.

Hi, I'm a LPN in Ohio and there is research that proves vitamin D3 if taken daily helps reduce falls. Our facility has taken this on and i have seen it work before my eyes. We had a gentleman that fell at least 3 times a day no matter what you did to prevent it. We did a Vitamin D3 level blood test and he had a deficiency. We started him on this med and it has helped. He has not fallen for the past two months. (knock on wood). We talked to the doctor and told him about the studies and he agreed to do it. Now everyone that is fall risk immediately gets a blood level draw of Vit D3 and if low we start them on the vitamin daily. It is prescribed in units. Hope this helps

At the nursing home i work in IN, we do a fall risk assessment upon admission. An alarm is applied when needed. Once during the shift and at shift change the alarms are checked by the nurses and aides.

preventing falls in long term, we do the following: Fall assessment upon admission then every week for a month then monthly. outside the door we put color coded dots, do labs for immediate change in behavior like confusion. bowel and bladder monitoring. falls comittee discuss things. physical therapy assessment. sensor alarm, bed bolster, w/c alarm, non skid footwear. monitor resident every 2 hours. behavior log book, charting.

Put the call light within the pt's reach and encourage them to use it if tgey are alert and oriented x3.Have a bed/chair alarm to notify staffs when they want to get up.Check them every two hours or sooner.Also an alarm on their w/c.Put a sign on their doors ,w/c.chair to indicate they are fall risk.Toilet them every two hours to reduce their chances of trying to get up.Have a foam matress on both sides of the bed and have the bed in the lowest postion at all times to prvent injury if a fall do occur.Savannahnurse i hope this do help.
