in the state of florida who is the that is respondiblie for patient care assessment the L.P.N. or the R.N.

I don't know about Florida. In Texas and Louisiana, RN's do head to toe assessments. LPN's aren't supposed to.In La., an RN could let an LPN do the assessment, but had to co-sign it. The RN was basically at that point taking responsibility for that assessment. So, if it wasn't right, they were taking a chance on their license.

Not sure for Florida but Pennsylvania LPN's can do  an assessement on a patient all except for heart and lungs and they can document on the patient again all except for heart and lungs ~~ Then the RN reviews the documentaion and  signs off on it and documents on anything that they  do not agree with. 


Our service to others is the rent we pay while here on earth!!

Initial assessment is done by an RN although an LPN can do it the RN must sign off so any RN worth their salt might as well just do it because who would sign off without checking the patient over anyways?

So the question comes up, is it a matter of facility policy, or is there a state law, JCAHO recommendation, Medicare Rule that requires it be done by a LPN vs RN.


Would be curious what generated this particular requirement.

In working in almost every state in the west it is my experience that the initial assessment is always signed by a RN. Others wrote that in some states an LPN can do this assessment but a RN must sign it regardless. I agree only a RN who is careless would sign anything done by anybody else and that includes another RN.

I'm becoming an LPN and plan to go to RN.  IAs an RN, I don't think I'd be willing to sign off on an assessment I didn't do.
