We have always done it that way

There are so many things in our profession that could be changed for the better. I know how difficult it is for people to accept change. Some of the things that we continue to do in nursing make no sense to me at all. For example: why in the world are we still forcing students to write care plans? Most hospitals do not use care plans as we once knew them and as we continue to teach. It is very important to teach the nursing process which the care plans are designed around. I think time would be better spent teaching students some assertiveness skills and leadership skills. Anyone agree with me about the stupid care plans? What else do you think we could change?

Yes. I had so many care plans to do. From beginning to end of school. Our instructors even told us they are not used like that anymore and doing our clinical rounds proved that. It doesn't make sense to keep teaching something that is now obsolete.

I'm so with you on the assertiveness and leadership skills. My hospital is sending me to a series on leadership because I was recently promoted. We did Myers Briggs inventory the other day. Great insights!

I have not written a care plan in a million years. But I wonder if they are helpful in the learning process for students.
And not having a "care plan" may not be good. What about special things that help individual patients? How do we communicate them? There is nothing worse for me than finding something that helps a patient, even if it a simple positioning trick to help them breathe better, and coming back on a following shift and finding nobody else tried it.
Or, managing a delirious patient without restraints and coming back to work to find them oversedated and restrained!

Indeed, the nursing process and critical thinking skills should continue to be taught. However, I believe that there are better ways of teaching than to have students spend hours on care plans that are not used. Care plans have outlived their time. Who can do long term goals when most patients are discharged in 24 to 48 hours? It is one of those we have always done it that way things. There are so many of the things we still do that need to go bye-bye.
