I am writing my first nursing care plan for LPN school. I am having difficulity with the rationale for this particular intervention of Assess the pt's decision making ability and orientation x3. The Nursing Diagnosis is Powerlessness R/T Health Care Environment AEB loss of independance. Can you give me some guidance to the rationale, i am stuck. Thanks.
