My friend just saw her CHF cardiologist and is now more confused than before. She made this follow up appointment because she was sob and just not feeling herself for few days and ended up going to the urgent care the second day of this incident. She was on her way home from work and suddenly felt very weak and she pulled into a parking lot and went into a cafe to get some water. While there she became nauseated, sob, she felt her pulse and it was pounding that is when she noticed the palpitations too. After about 40 mins it seemed like things were calming down so she decided to drive home and as soon as she began driving everything started again. That's when she decided to go to the nearest place which was the urgent care. They did an ekg, chest x-ray and said things looked okay but wanted her to go to the ER or call her doctor right away because the pacemaker can make everything look normal.  The urgent care doctor said the blood may be pooling in the heart and they do not have any way to determine if it is and could not do blood work. My friend said everything calmed down so she went home. She shared all this with her cardiologist and she seemed angry and suggested she get a second opinion. The whole time she is writing a page of notes and checking off several boxes on her form and ask when my friend was going to the pacer clinic again. My friend thinks her doctor was having a bad day and frustrated. Do you think the Cardiologist knows what her problem is and is not telling her because her symptoms are getting worse?

I think the cardiologist would have said something if she thought the patient was getting worse.  It's hard to tell what really happned between the patient and the doctor.  She should have gone to the ER as suggested; I wonder why the urgent care didn't insist on calling 911.

But there is something missing from the story.  Your friend must have said something for the cardiologist to suggest she get a second opinion.

In any case, if she has these symptoms again she should call 911 and go to the hospital right away.  SHE SHOULD NOT DRIVE if she has these symptoms; she could pass out and cause a car accident, and kill either herself or someone else.

Didnt anyone get a BNP on this woman? Pretty standard stuff for pt with CHF. Also what is the patients underlying rhythm as you mentioned a pacer and has her pacer been interrogated lately? Obviously a few different things going on here that an Urgent Care center just isnt capable of handling. As far as the Cardiologist being pissed off.....that is their general demeanor. I have  worked with many Cardiologists and most of em, although they know their stuff, have the bedside manner of a Cobra. JMO
